Care Coordinator

**Overview**
**Responsibilities**
- Coordinate care services for clients, ensuring that their needs are met in a timely manner.
- Serve as the primary point of contact for clients and their families, providing support and guidance throughout the care process.
- Maintain accurate records of client information, care plans, and service delivery.
- Collaborate with healthcare providers to develop and implement personalised care plans.
- Monitor client progress and adjust care plans as necessary to ensure optimal outcomes.
- Conduct regular follow-ups with clients to assess satisfaction with services provided.
- Assist in the training and support of new staff members within the office environment.
- Ensure compliance with all relevant regulations and standards in care provision.
**Experience**
- Previous experience in a healthcare or social services setting is preferred but not essential.
- Strong organisational skills with the ability to manage multiple tasks effectively.
- Excellent communication skills, both verbal and written, with a focus on building relationships.
- A compassionate approach to client care, demonstrating empathy and understanding at all times.
