We have a fabulous job opportunity for you to join our awesome team as a Support Coordinator. We support our brilliant customers who can be challenging at times, but are hugely rewarding to work with
Our customers are 18+ and at risk of homelessness. Almost 80% of them have a mental health diagnosis and we help them with emotional support, practical support, advice and advocacy.
They live at Hawthorn House, a beautiful listed building in the Elswick area. There are 10 rooms in the main building with shared facilities and 25 customers living in the community in single occupancy supported tenancies.
We help our customers to change their lives, and support them to work towards achieving their aspirations. We deliver the support that has been identified, in the individual plans that have been created in partnership with our Support Coordinators, clinicalteam and customers. What we can tell you is that each day is totally different, so if you’re looking for a calm stable role, then this is not the job for you
If you thrive on a challenge where you can influence a young person to make lasting changes to their lives, then keep reading One day you might be cooking with customers, supporting them to shop on a budget and prepare healthy meals and the next you mightbe supporting them at a probation or substance abuse support meeting
You’ll work in partnership with our external partners and with your Home Group colleagues to assess and engaging support for your customers. Together you’ll really make a difference, and work towards helping your customers successfully move on with a positivefuture.
