**Pay rates**:
Home Care Assistant pay rate is £15.50 and £16.50 an hour. Bank holidays are paid at an excellent rate of £30 to £32 an hour

**Job Summary**
The Senior care assistant plays a vital role in ensuring that patients receive comprehensive and coordinated care. This position involves working closely with healthcare providers, patients, and their families to facilitate effective communication and support throughout the care process. The Senior care assistant will be responsible for managing patient schedules, maintaining accurate records, and providing administrative support within an office environment.

**Duties**
- Coordinate patient appointments and ensure timely follow-ups to enhance continuity of care.
- Maintain accurate and up-to-date patient records, ensuring compliance with confidentiality regulations.
- Act as a liaison between patients, families, and healthcare professionals to facilitate effective communication.
- Assist in the development of care plans tailored to individual patient needs.
- Provide administrative support including data entry, filing, and managing correspondence.
- Monitor patient progress and report any concerns to the appropriate healthcare team members.
- Participate in team meetings to discuss patient care strategies and improvements.

**Experience**
- Previous experience in an office setting is essential, preferably within a healthcare environment.
- Strong organisational skills with the ability to manage multiple tasks effectively.
- Excellent communication skills, both verbal and written, to interact with diverse individuals.
- Proficiency in using office software and electronic health record systems is advantageous.
- A compassionate approach to patient care with a commitment to improving health outcomes.
