**Interim CHC Assessor**

My client, an ICS based in the North of England is looking for an accomplished, motivated and dedicated interim CHC assessor

Day to day duties:

- Hybrid role with face-to-face assessments
- Complete assessments, checklists, DSTs, and Fast Track Reviews
- Determine eligibility of NHS CHC funding through new and review assessments
- Manage own case load of CHC cases ensuring timely delivery
- Liaise with multidisciplinary agencies in relation to Continuing Healthcare
- Collated information for MDT meetings
- Work with families, care providers, social workers, and patients
- Make recommendations and write up cases to be presented to panel meetings

**Personal requirements**:

- Proven track record of performing high level CHC assessments
- Strong communication and stakeholder skills
- Committed and dedicated to maintaining patient care
