Witney, United Kingdom Oxford Health NHS Foundation Trust Full time

We're looking for a team player who wants to make a difference in providing sub acute care in the community

Witney Emergency Multi
- Disciplinary (WEMU) provides sub acute assessment and treatment for patients with a holistic focus on supporting them to remain at home, avoiding unnecessary hospital admissions. We deliver our service with an MDT consisting of a varied professional group who work as one team.

The HCA role is to work with a specialist Multidisciplinary Team (MDT) providing effective assessment, therapeutic patient care and discharge planning in a sub-acute medical setting. The role encompasses a mixture of general and specialist assessments under supervision by a registered practitioner within Trust policies, protocols and guidelines and in accordance with the unit philosophy.

If you are enthusiastic, committed and thrive on patient focused care we would love to hear from you.

As healthcare assistant working in WEMU you will be an integral part of the clinical team, contributing to the assessment and treatment plans for patients. You will be working collaboratively with the multidisciplinary team to provide hospital level care to patients on the unit and in their own homes.
