CHC Band 6 Assessor

We are currently working with a reputable CCG based in North of England. The CCG are looking for an enthusiastic and highly organised individual, to provide a first-class support to the Continuing Healthcare Team
- You will have some CHC assessment experience and have strong communication skills, both verbal and written
- Knowledge of Continuing Healthcare required
- You will have experience of carrying out a range of needs based assessments.
- You will need to be organised and confident to work alone and as part of a team, keen to learn new ways of working and happy to take part in training in order to develop assessment skills
- The ability to travel, to manage your workload and to work flexibly in a pressurised environment is essential

Main duties of the job

As a Continuing Healthcare Nurse Assessor, you will work under the guidance and support of the senior clinical team leads to implement the National Service Framework for NHS Continuing Healthcare and NHS Funded Nursing Care for residents in North of England.You will promote best practice and provide professional advice to nursing homes and other providers, promote good relationships and be able to liaise with colleagues from other disciplines both internally and externally to the Clinical Commissioning Group.

Be able to show a robust approach to problem solving and the ability to prioritise your own workload. You will need to have a flexible attitude and be a keen team player who enjoys working within a busy environment.
