**A fully qualified ACP must be able to**: Complete a full system and physical or psychological/psychiatric exam Take a systemic and/or psychosocial patient / prisoner or service user history Make diagnostic decisions based on consultation, history, presentation, pathology and/ or imaging results Initiate, prescribe and monitor the impact and outcome of treatments and medication including efficacy, side effects, contraindications and risk Devise an individual clinical management plan that includes all aspects of multi-disciplinary care Screen patients for early signs of disease and risk factors and relapse indicators Carry out where appropriate, invasive and non-invasive diagnostic and therapeutic procedures with due regard to mental capacity and advance decisions Refer to other health professionals as appropriate Discharge from hospital or services as required Run independent out-patient clinics seeing new and review individuals as required For trainee ACP the requirements include To undertake appropriate training in order that the following clinical skills can be achieved: The ACP is accountable for independent clinical assessment, formulation, diagnosis and treatment of these individuals with undiagnosed and undifferentiated conditions. They will provide expert professional and independent clinical care which enables the coordination of a multi professional seamless service. Practise in compliance with respective code of professional conduct and within scope of practice, being responsible and accountable for their decisions, actions and omissions Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information. Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change.

Work in partnership with individuals, families and carers, using a range of assessment methods (e.g. of history-taking; holistic assessment; identifying risk factors; mental health assessments; requesting, undertaking and/or interpreting diagnostic tests and conducting health needs assessments). Demonstrate effective communication skills supporting people in making decisions, planning care or seeking to make positive changes, through person-centred approaches in health and care Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses. Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.
