**Job summary**:
Following the change in stroke provision in the Somerset we are excited to be able to expand our Advanced Care Practitioner workforce and are pleased to advertise 3 ACP / ACP training posts in acute stroke in Somerset NHS Foundation Trust

Within Somerset NHS Foundation Trust there are two acute hospitals and whilst the expectation is for you to be based primarily in one, which will be discussed at interview, there will be times when working or training at the other site will be required. There are 2 posts available on the Musgrove Hospital site in Taunton which will be providing HASU, ASU services and out-patient services and 1 post on the Yeovil Hospital site which will be providing ASU and out-patient services.

We have a robust team of medical consultants who have experience as being supervisors in the ACP training pathway and there is dedicated training time in addition to the university study days to ensure that the trainee is appropriately supported throughout their academic journey.

When the service is fully established the anticipation is that the ACP provision at MPH will be 8am to 10pm and at YDH 8am to 6pm.

**Main duties, tasks & skills required**:
This role is very clinically focused with the expectation that you work closely with medical colleagues in the initial assessment of the patient in the Emergency Department, routine daily ward rounds and run follow-up clinics for both Stroke and TIA patients. For the Musgrove Park site there is an expectation to be actively involved in thrombolysis calls and referral to North Bristol Hospital for thrombectomy. For the Yeovil Hospital site, whilst thrombolysis will not be routinely administered it is essential that the postholder is up to date in acute assessment should there be an in-patient stroke or patient self presenting to the hospital.

There is an expectation that you will support junior colleagues and students and both sites are very research active.

Somerset has an aging population and as a result a focus on primary and secondary prevention is essential.

Somerset is undergoing a stroke review and as a result will work very closely with Dorset County Hospital and it is essential that there is an equity of provision for all patients so the postholder would be actively encouraged in being involved in not only audit, education, but also improving cross county relations.

**Essential**:
Evidence of BSc level education, such as:

- NMC, GPhC or HCPC
- BSc Advanced Practice.
- Equivalent / other BSc level mapped to enhanced practice.
- Agree to undertake and complete as MSc Advanced Practice or achieve equivalent to the national standard
- Evidence of Continuing Professional Development
- Relevant role specific qualifications (ie swallow screening, thrombolysis training NIHSS)

**Desirable**:

- Non
- Medical prescribing if appropriate to registered profession/role
- Leadership/management course or validated portfolio evidence

**Experience**:
**Essential**:

- Significant experience of working as a registered practitioner in a provider organisation (e.g., acute sector, community provider, third sector, social care or primary care) and within a relevant specialty (4 years minimum)
- Stroke experience in line with Stroke Specific Education Framework (SSEF) requirements for Band 6/7
- Demonstrate an up to date and detailed knowledge of relevant guidelines and clinical practice in the specialty.
