**Job Summary**:
Provide line management to the Care Coordination team, support delivery of the Digital Transformation agenda, support the senior leadership team and provide support services to PCN practices to ensure excellent patient care.

Act as a General Practice Team Leader when required, providing high quality leadership to practice staff in line with PartnersHealth operational objectives

**Job responsibilities**:

- To lead & line manage the Care Coordination team across the PCN
- To work with the Digital Transformation Lead & PCN Service Manager to deliver the digital transformation priorities for the PCN playing a key role in bringing together practices & supporting implementation & roll out of objectives
- To establish & lead meetings across the PCN to support this: establishing care coordinator, administrator and reception and medical secretary forums
- To work across and within practices as & when required to support operational sustainability
- Provide cover as Practice Team Leader when required
- To work across all Rushcliffe PCN practices to support implementation of change

**Management of staff**:

- To lead & line manage the Care Coordination team across the PCN
- To lead & manage staff within a practice as required by the PartnersHealth business model supporting practices
- Organise staff rotas, appointment books and annual leave
- Ensure an adequate and safe level of staff through rota and management planning
- Assist in monitoring and evaluating performance of staff against objectives, help to identify and manage staffing and service changes to ensure
- Monitor relevant day to day administration, in relation to patient services ensuring it remains effective
- Maintain effective communication both with the Practices and all the relevant agencies
- Arrange team meetings; prepare agendas, take minutes/action notes and distribute as necessary
- Assist the Practice Manager in the implementation of all local and /or national enhanced services where the Practices are participating
- Assist in the development of protocols, review and update as required in conjunction with the PartnersHealth management team
- Organise and manage meetings to promote good team working across the practice teams
- Liaise closely with all other members of the PartnersHealth and Primary Care team (internally and externally)
- Take responsibility for recruitment & selection, supervision, appraisals, organisation of team annual leave, and be the first line contact for sickness, disciplinary etc

**Information Technology**:

- Liaise with NHIS to solve IT problems and ensure a smooth running of practice information systems
- Provide support and expertise to staff across PartnersHealth for SystemOne, smartcard and other clinical access such as ICE across a range of services and Practices

**Patient Services**:

- Ensure compliance with patient confidentiality at all times, in line with current legislation
- Working with the Practice Team, ensure the relevant patient services are provided and maintained
- Ensure all patient information is accurate, easily accessible and kept up to date
- Participate in regular audits across all services regarding patient satisfaction conducted in conjunction with the reception team and phone system audit
- Be the first point of contact of for complaints and resolve as appropriately, ensuring relevant reporting of complaints and incidents
- Initiate, develop and maintain excellent third-party relationships, e.g.: patient groups and other stakeholders to develop, maintain and support services and performance delivery
- To develop understanding of all current PCN services e.g. Enhanced Access, Mental Health Practitioners, Spirometry, Digital Inclusion & ensure care coordination support to these teams is optimal
- To develop an understanding of primary care and PCN priorities, such as the Impact and Investment Fund indicators & processes required to support monitoring of achievement
