We are currently looking for additional compassionate, caring and dedicated individuals to join our team of Community Care Workers. We cover homes in **Barnstaple, Bideford, South Molton **and **surrounding areas. **As a Community Care Worker, you will be responsible for providing care and companionship to our clients in their own homes; following personalised care plans to meet each person's specific needs and preferences.

You'll have the flexibility to work from **07:00 to 14:30 **and/or **16:00 to 21:30** in accordance with an agreed **contracted shift pattern **and will be assigned a centralised area to work within. You must be able to work every other weekend. This is a great opportunity to work in a fulfilling and rewarding career while making a meaningful impact on the lives of others.

**Responsibilities**:

- Providing personal care and support to clients, including assistance with bathing, dressing, toileting, and grooming
- Helping clients with mobility and transferring, as well as with eating and drinking
- Providing emotional support and companionship to clients
- Assisting with medication management, as directed by healthcare professionals
- Monitoring and reporting any changes in clients' health or wellbeing
- Communicating effectively with clients, their families, and healthcare professionals
- Maintaining accurate records and documentation of client care
- Complying with all Chapter Care policies and procedures, as well as all relevant legislation and regulations

**Requirements**:

- A compassionate and caring nature
- Excellent communication and interpersonal skills
- The ability to work independently and as part of a team
- A flexible and adaptable approach to work
- Previous experience in a care-related role is desirable but not essential
- A full driving license and access to a vehicle is preferred
