**Family Support Worker**

**Background and Service Specifics**

**Sandwell Children's Centres**:
Barnardo's is proud to be commencing delivery of Sandwell's Children's Centres across the Borough. The service will provide a range of quality services for families, locally designed for the 6 town areas of the Borough, divided into 3 areas:

- Cluster A - Oldbury, Smethwick
- Cluster B - Tipton, Rowley Regis
- Cluster C - Wednesbury and West Bromwich

**Across the cluster, we are committed to delivering a robust offer for communities and residents, designed to promote access to services, reduce barriers to engagement and build resilient communities.**

The Family Support team will take an active lead to working with partner organisations, local communities and residents to achieve best outcomes for the families within that area(_for the purpose of this JD parents includes parents, carers and guardians)_.

**Objectives/key activities - Family Support Team**
- Plan and support delivery of targeted service elements, producing KPI's and outcomes i.e:

- Complete Early Help Assessments in conjunction with families.
- Act as Lead Professional where families are being supported at Universal threshold and complete Team around the Family meetings.
- Caseload management of up to a maximum of 15 families, depending on complexity and intensity.
- Completion of high-quality Outcomes Stars with families to highlight strengths, show and monitor progress and measure the impact of service in line with service specification.
- Working Together with Families to provide practical day to day support with families; developing practical parenting skills and strengthening the functioning of the family.
- Complete the Graded Care Profile 2 for families (where appropriate).
- Reducing Parental Conflict utilising tools and strategies where appropriate.
- Support the delivery of Parenting/Family Support Interventions.
- Referrals to Sandwell Consortium and other agencies as necessary to meet the family's individual needs.
- To strengthen families in a trauma informed way; promote resilience and self-care to prevent children and young people requiring more specialist and intensive support through high quality and impactful direct work.
- To promote independence, self-confidence and empowerment for parents, children and young people to make informed choices.
- To support families to access universal services as appropriate.
- To capture the voice and experience of children and young people and utilise this to inform and develop service delivery.
- To ensure the involvement of fathers and significant males.
- To work in partnership with Sandwell Children's Trust and Strengthening Families Service.
- To offer a range of other evidence-based interventions to support their work with families.
- High quality case recordings completed on Sandwell's EHM system for all families being supported who have given consent, whatever the level including universal, universal plus, children in need or child protection. Ensuring timescales are met.
- Complete and upload all relevant forms (risk assessments, Consent, GDPR, CC registration form).
- Work with the wider Children's Centre teams, including support for volunteers, to deliver activities and service specifics.
- To inform Education Engagement worker of children identified as SEND within Children's Centre Services.
- Working in collaboration with agencies to strengthen the exit strategy to ensure sustainable change.
- To actively engage in reflective case and personal supervision, PDR's and be responsible for updating learning, implementing into practice and develop skills to meet service needs.
- To support students/volunteers and new team colleagues as and when requested by line manager.
- Work with the wider teams within the Children's Centre Offer to meet the needs of the service across the borough.
- To meet the needs of the service evening and weekend work will need to be undertaken as required.
- Any other duties deemed appropriate within your job role to reflect the needs of the service.
