**Community Hub Co-ordinator**:
An exciting opportunity has become available at Create Community Hub as a Community Hub Coordinator. Create is an unique service, provider for adults with mild to moderate learning disabilities. The post holder will work within a team to deliver a person-centered Community Hub that promotes the independence of adults with learning disabilities and supports their carers. This post will support and work in partnership with people with learning disability to develop a range of activities which will promote independence, employment opportunities, social skills and health and well-being. Act as a keyworker for a number of service users taking into account their physical, psychological, cultural, social and religious needs. Assist users in all aspects of personal care and hygiene to ensure a high quality, culturally appropriate service to vulnerable people presenting with a range of support needs
