Outreach Support Worker

The post holder will undertake all the core tasks within the remit of the residence to provide for the physical, health, emotional and social needs of the service user, including promoting independence, whilst preparing, assisting and enabling them to fulfil the aims of their Person Centred Plan.
The post holder will be working in one or more service user’s home. The main focus is to provide the highest level of support in order to enable independence and empower services users to control their own lives. To achieve this, the service provides extensive care and support and actively promotes independence and inclusion in the community.
The service users will have a variety of complex needs which may include Autism spectrum conditions, mobility, communication, health and social care needs.
Main duties will be carried out working shifts allocated by the line manager, which may include weekends, evenings and sleep-in duties. As part of this role the post holder will be expected to support service users in their homes and within the community, which may include general outings, holidays or appointments they are due to attend.
A number of administrative hours will be spent attending meetings, supervision, annual appraisals, training, and service reviews.
The post holder is expected to complete an appropriate Induction programme and to fulfil the Domiciliary Support Worker accountabilities to a standard complying with the National Domiciliary Care Standards for the post.
At all times, the post holder is expected to conduct and attire themselves professionally, to work within the current legislative framework and adhere to company policies and procedures, in particular the Safeguarding of Vulnerable Adults. The professional behaviour expected also entails bearing in mind that the post holder’s job takes place in people’s homes, and that at all times their wishes and rules should be respected.
**Responsibilities**:
- Ensure the provision of care & support to clients according to their individual support plans.
- Support the development and maintenance of the individual skills of clients as per their plans to include learning and skills activities, engaging in a person centred approach.
- To ensure that up to date records are maintained and to provide written reports as required
- Support clients to meet their domestic and personal needs.
- To ensure that all in-house financial transactions are conducted and recorded according to policies and procedures.
- Support clients to access and participate in recreational activities in their immediate locality and wider community.
- Relate to and interact with Clients.
- Ensure your actions support equality, diversity, rights and responsibilities of Clients.
- Maintain a safe and clean environment.
- Protect yourself from the risk of violence at work.
- To participate in NVQ Programme
- To participate in six weekly supervisions with designated supervisor.
- To undertake all statutory and mandatory training.
- To undertake additional identified training as appropriate and in agreement with the line manager.
- Any other duties commensurate to the post.

Pay: £11.44-£12.00 per hour
