Our Urgent Care Hub is a collaboration of our Crisis Resolution Home Treatment Team and our Haven team working together and across the urgent care service. These services are integral to our Urgent Care Pathway and ensure people presenting to acute mental health services receive a compassionate, person
- centred assessment and timely care plan whether this occurs in The Haven or with CRHTT in the person’s home.
We are looking to recruit skilled and highly motivated practitioners to join us as Urgent Care Practitioners, working closely with The Haven, the acute hospital and community teams to support people. We are specifically looking for practitioners experienced in carrying out specialist assessments and offering treatment interventions to people presenting with co-existing mental health and substance misuse issues.
CRHTT are able to offer all the benefits of a hospital admission whilst remaining at home, including visits from our experienced multi
- disciplinary team. We facilitate supportive discharge from hospital, managing the transition back to a community-based service. We are committed to a focused recovery model.
You will be an excellent communicator, demonstrating empathy and facilitating an inclusive and collaborative environment and assessment to those service users accessing either/both The Haven and CRHTT services with a particular focus on:
- Utilising your excellent clinical skills and judgement to assess individual needs and risks
- Ensuring that high standards of assessment and treatment plans are provided to people with dual diagnosis issues, based on the best available evidence.
- Maintaining strong links with local substance misuse services and make onward referrals as required.
- Demonstrate an understanding of clinical interventions relevant the care group to include education, care plans and supportive discharge planning,
- Support the wider team in the management of dual diagnosis service users, including delivering bitesize training
- Work compassionately and creatively to identify therapeutic and evidence-based outcomes and care plans for people who are experiencing a crisis in their mental health
- Work closely with carers, families and partner agencies to facilitate and promote the safety and the well-being of service users and their families/carers.

We provide mental health and learning disability care for children, young people and adults across Sussex and for children and young people in Hampshire.

This is an exciting and dynamic role that offers the opportunity for development and implementation of high-quality services. Previous experience of working in acute services or home treatment teams is desirable, although not essential. We offer excellent staff support through clinical supervision and teamwork.
As our Coastal Urgent Care Pathway moves towards CORE 24 standards, the post holder would need to work flexibly to meet the needs of the service which could include working unsociable hours within a 24/7 service. This role supports the work of and across Urgent Care Teams (CRHTT, The Haven, Mental Health Liaison team and the Liaison Suite at Worthing General Hospital) and as such the post holder may be expected to work across these teams as required.
Working unsocial hours does bring a salary increase, in line with Agenda for Change guidelines.
