This is an exciting opportunity for a Band 3 support worker, to be part of the Dementia and frailty pathway working in our Mid Essex PCN (Primary Care Network) team, supporting the diagnosis of dementia. This involves working alongside the team with GP practices.

The role is Monday-Friday 5 days a week 9-5 and it is essential that you can drive and have access to a car, as you will be working across Mid Essex - Maldon, Braintree and Chelmsford.

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year, working shifts where appropriate and operationally required

Please note it is essential criteria that you have a UK driving license and access to a car for work purposes

Good communication skills, and being IT literate and computer skills, you will be trained in physical health as the role includes: assessment, observations, ECG’s, and phlebotomy.

It would be beneficial having experience or understanding of working within a team, at times you will be required to work without the supervision of others, and you will need to have the ability and adapt as the service requires.

Having the care certificate is a requirement. However this can be undertaken in the role if you are successful in employment.

As part of the Band 2 HCA role you will be enrolled onto the Apprenticeship scheme, providing you with the opportunity to earn as you learn. This role enables you to gain valuable experience on the wards/community settings whilst working towards gaining your Care Certificate and naturally progressing into the guaranteed Band 3 HCA role.

What is Staff Bank?

Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

Working with individuals within the dementia and frailty pathway, you will be responsible for assisting with the delivery of patient centered care under the supervision of a senior member of the team; providing practical support to service users in performing day to day activities, guidance and education, and in assisting in their management and care based upon individual needs under the guidance of a senior member of the Team.
- Assist in the management and care of service users under supervision
- Produce accurate and timely documentation / records on all clients in line with relevant codes and Trust policy
- Assist senior staff in the ongoing assessment and observation of service users and provide timely and appropriate feedback
- Provide practical support to service users, carers
- Transport / accompany service users to appointments
- Attend Team meetings i.e. referral / review / business as appropriate
- Report and document incidents as required
- Undertake physical health care checks of service users, document and report results
- Under supervision take part in assessing and reviewing care
- Provide support to service user to promote concordance with medication and treatment regimes in line with Trust policy
- Transport and store medications in line with Trust policy
- Provide practical support, guidance and education for service users, carers
