To work under the direction and supervision of qualified nursing staff in the provision of nursing care to individuals who meet the CRT criteria in the community setting.
To support the team with the provision of nursing care within the community setting. To undertake delegated tasks without direct supervision but reporting findings back to the Clinical Nurse Lead
To undertake basic nursing care duties for the team
To work alongside social care staff where necessary.
To function as a member of the CRT
To assist the qualified nurse in the provision of nursing care.
To undertake identified nursing duties under the guidance and supervision of a qualified nurse although often working without direct supervision.
To provide care consistent with the Primary Care Directorate Nursing Standards.
To undertake mandatory and developmental training as directed by the Clinical Nurse Lead
To participate in annual PDR and identify personal and team objectives.
Neath Port Talbot Acute Clinical Team is a dynamic Nurse Practitioner Led Service supported by Consultant Geriatricians.
The Service covers a vast Geographical area
- in order to fulfil this role, the successful applicant must have access to a vehicle.
The Role of the Band 3 HCSW includes
Ability to recognize acutely unwell adults and liaise with the Duty Nurse Practitioner in order for treatment plans to be commenced by suitably qualified staff.
Phlebotomy Skills
Collection of Microbiology Samples with appropriate documentation
Early Life support competent
Ability to perform Point of Care Blood/ urine testing as per Health Board Policy (training provided)
12 lead ECG's and bladder scan skills
Simple wound care
Pressure ulcer prevention and reporting of such - accessing equipment, liaising with Nurse Practitioners and Nurses within ACT
Awareness and recognition of venerable adults and how to escalate concerns
Correct storage and documentation of patients notes as per Health Board Policy
Always maintaining Professionalism promoting the Service Positively to Service users and other Healthcare Professionals
Attending internal/ external training to enhance personal skills and experience
Reporting of incidences via DATIX (with training)
Maintaining confidentiality of service users at all times
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
