This is an exciting opportunity for a Mental Health Practitioner who has an interest in the frail elderly. This Band 7 Clinical Lead post is placed within the Care Home Support Service (CHSS) team. The CHSS isa multidisciplinary team which consists of specialist nurses, physiotherapists, occupational therapists, healthcare assistantsand mental health occupational therapists/nurses. A medical consultant, and psychologist give clinicalleadership to the team. The CHSS works in partnership with care home staff in nursing and residential homes and aims to support them to improve the care of their resident across Oxfordshire and to avoid unnecessary hospital admissions for their residents. The post forms part of this team and provides a highly skilled evidence-based model of care for those residents who are experiencing behavioural and psychological symptoms of dementia. Applicants will be able to demonstrate an advanced knowledge of older people's mental health and the complex associated comorbidities. The CHSS MH team works with people experiencing organic mental health needs and evidence working across both physical and mental health groups is essential. Applicants should be experienced in managing high levels of risk while maintaining safe practice for individuals and the team. You will be able to provide examples of clinical leadership within a caring and supportive environment. Applicants will be able to demonstrate post qualification education or equivalent knowledge and be driven to further develop knowledge and skill. A passion for older adult's mental health and service improvement is an essential quality for this post. As a service the CHSS continues to drive improvement to provide the best outcomes for the people who access the team. Main Purpose of the Post 1. To lead and manage a small team of mental health clinicians, providing a high standard of professional service to patients, in alignment with national and professional guidelines. 2. To perform therapeutic assessments of patients with diverse and complex presentations and physical and psychological conditions, to provide and formulate an individualised intervention plan with the care home staff. 3. To undertake all aspects of clinical duties as an autonomous practitioner, with professional and legal accountability for all aspects of work. 4. To be responsible for own caseload and that of other team members. 5. To actively manage referrals from the Community Mental Health teams. 6. To participate in regular formal training and clinical reasoning sessions, peer review and case conferences.
