We are looking for a Band 4 Community Nurse Associate to work within the Cambridge City North Neighbourhood Team supporting patients with clinical health needs.
As a nursing associate, you will be working within the multi-disciplinary team and neighbourhood team. You will provide high standard of nursing care and interventions within the patient’s own home, with support from the wider team. The patient will be at the centre of the care being provided, ensuring patient and carer participation in decision making.
We provide a service seven days a week, working closely with other community services and PCN's to provide high standard care.
You will have the opportunity to develop yourself, with extensive learning and development opportunities.
**We are ideally looking for people who have**:
- Experience of community nursing, palliative care, care of patients with diabetes
- Educated to Registered Nursing Associate on the NMC Register
- Experience in falls prevention work or willing to gain skill
- Experience of caring for and supporting adults and older people
- Experience of assessing patients and formulating care plans independently and where appropriate, with support from RNs in the team.
- The ability to demonstrate a commitment to high quality service provision
- The ability to work flexibly and as part of a team, as well as individually
- The ability to travel independently using own vehicle
- Good time management and the ability to manage individual workload
- Enhanced communication and inter personal skills

- To ensure that accurate and timely written records are kept which comply with the Trust policy and NMC guidance, reporting on any issues as appropriate.
- To provide skilled nursing care of a high standard as delegated by the district nursing sister/ charge nurse.
- To support, educate and advise formal and informal carers with the nursing care which they can provide.
- To attend all relevant mandatory training as and when required to do so.
