We are currently recruiting for the below post:
**Social Worker Adults Older Person’s Team Milton Keynes. Immediate start with ongoing contract. Paying £32.00 per hour**
- You will need to have strong post qualified experience in Children’s Team - Assessment, Duty work - 2 years minimum qualified experience desired.
- Completing Social Care initial assessments and reviews of needs for adults/older people in hospital settings, residential and Nursing placements and in clients own home within the community.
- Completing Mental Capacity assessments in line with the Mental Capacity Act 2005 for specific decisions, for example change of accommodation, financial management and personal care, consent to complete NHS \continuing Healthcare.
- Completing Court of Protection Documents as part of outcome of assessments as and when required.
- Identifying safeguarding and undertaking safeguarding investigations and leading safeguarding enquiries following the guidelines as outlined in the Care Act 2014
- Maintaining case load, care management, dealing with complex cases and working within a timely manner to meet deadlines.
