**Specialist Care Coordinator Mental Health**
Monday to Friday 8 - 4 (some flexibility required)
The main responsibilities will be to:
- ensure the people in your service are provided with evidence-based mental health care and this is co-ordinated to meet their individualised needs
- attend complex case meetings and professionals meeting as required and contribute clinical expertise to support the development of the PD/ED service.
- work within and adhere to the personality disorder pathway.
- work within the current CPA guidelines by designing, delivering and evaluating care plans and risk assessments to meet the needs of individual clients.
- assess under the new care plan approach
- support the service user to develop and adhere to all aspects of their care plan bed and work with them to understand what is working and what is not
- to provide psycho-education for the eating related difficulties.
Applicants should be an experienced Mental Health Nurse, Social Worker or Occupational Therapist with specialist knowledge of working with personality disorders or eating disorders. Post registration training in eating disorder and/or Dialectical Behaviour Therapy would be beneficial.
