Rehabilitation Assistant - Band 3 - Full time position

We have an exciting opportunity for a dynamic Rehabilitation Assistant to join our Islington urgent and rapid response service.

We are a community based team providing multi-disciplinary assessment and treatment to adults and elderly across the borough of Islington. We are a dynamic team that enjoy working together to establish a highly responsive and effective service.

The post holder will work in the Islington urgent and rapid response service.Integrated with therapy and social services colleagues, the service aims to respond to patients in the community for urgent response.

Under the supervision of a senior therapist, you will act as a generic worker providing vital support to therapists (e.g. Physiotherapy, Occupational Therapy, Speech and Language therapy) within the team to deliver and provide interventions that support clients with physical, cognitive and psychological needs. You will be involved in implementing interventions such as supporting a person’s rehab goal and improving independence in the community.

Clinical
1. To independently complete assessment of clients e.g. falls assessments/reviews and mobility reviews, and report back findings senior team member. In regards to mobility assessments managing difficult and aggressive clients, administration, liaising with other professionals and ensuring the data base is updated.
2. Work independently with clients with complex conditions including physical, cognitive, inter and intrapersonal impairments.
3. To feedback findings of these assessments to the team in the MDT meeting and to individual therapists.
4. Achieve competencies in Occupational Therapy, Speech and Language Therapy, Physiotherapy and Psychology.
5. Facilitate the smooth running of services through a flexible and adaptable approach to work.
6. To independently complete risk assessment of environment and safety following local and Trust policies.
7. To obtain information from clients regarding their interests and skills. Independently encourage and support clients in participating in activities previously undertaken. Feeding back the findings to the therapist.
8. To independently treat clients at home and other community settings, following guidelines from senior team member.
9. Under guidance of Speech Therapist, assist with making and developing communication aids such as communication books and charts.
10. To independently complete graded exercise / rehabilitation programmes following guidelines from senior team members. This involves adapting basic treatment programmes.
11. To independently complete graded outdoor mobility programmes with clients. This will involve continuous assessment of environment and potential safety risks. This includes public transport practise and accessing local leisure facilities.
12. To assist in completing group activities with clients (exercise groups / communication groups, vocational rehabilitation groups). This will involve set up of equipment, administration tasks, assisting senior team member and also independently running sections of the group activities.
13. To teach and demonstrate complex skills to clients e.g. using mobility aids.
14. To demonstrate and teach exercise / rehabilitation programmes to carers and report back to senior staff members.
15. To monitor above mentioned exercise / rehabilitation programmes and report back to senior staff members.
16. To engage in and provide training to colleagues at all levels.
17. Order specialist equipment and measure for specialist equipment to be fitted.

Education and development
1. To be responsible for completing objectives / activities set in yearly appraisal and supervision scheme under guidance of senior team member.
2. To maintain own Life Long Learning portfolio and keep up to date with current and relevant developments in area of practise.
3. To activity participate in staff meetings and in-service training programmes, including those related specifically to Mobility assessment.
