**Job overview**:
In supporting the Head of service you are responsible for developing the Hospital based Transfer of care team and working across the community discharge hub to improve pathways. The post holder will be responsible for the continued development and implementation of the Discharge to assess process responding to the Government agenda request to deliver Care close to Home.
The post holder will have ongoing responsibility for the Discharge to Assess process and supporting staff to safely manage the transition from hospital to home as well as preventing unnecessary hospital admission to maintain service delivery through the urgent care system.
Delivering a Transfer of care discharge hub and community response team to support the hospital avoidance agenda.
The post holder will ensure the provision of holistic and highly skilled assessment and rehabilitation by an experienced multi-disciplinary team to adult patients. This will form the basis of rehabilitation and discharge planning, the prime reason for admission. The post holder will ensure the coordination of defined teams implementing and maintaining high quality services in partnership with internal and external stakeholders.
**Main duties of the job**:
- To ensure effective and efficient operational management of the Discharge to assess and urgent care services across Oldham.
- To undertake overall responsibility for the clinical and professional leadership within the discharge to assess agenda in order to ensure safe and effective care delivery.
- Working in collaboration with the system partners.
- To provide specialist knowledge to the organisation in relation to the Discharge policy.
- To be accountable for strategic planning of the Discharge to assess agenda.
- Ongoing development of service contribution in the light of patient feedback and national policy developments and recommendations
- To maintain effective communication links between all teams within Community and Hospital services, working in collaboration with the wider services, secondary care and commissioners ensuring a standardised approach to the communication networks and systems established.
- Contribute to ongoing service planning of discharge to assess in line with local and national policies, targets and capital developments, providing professional advice and support in line with evidenced based practice and clinical effectiveness thereby supporting staff working across services.
- To provide specialist knowledge to the discharge to assess agenda, ensuring patient care is of the highest standard.

- To further develop multi-disciplinary team and inter agency alliances and effective working practices to support Urgent care contribution to pathways of care promoting care closer to home care and long term conditions management.

- To provide specialist knowledge to the organisation in relation to the Discharge policy.
- To be accountable for strategic planning of the Discharge to assess agenda.
- To ensure specific key performance indicators are managed and service performance is assessed.
- To assist the Senior Manager in planning, implementing, delivering and evaluating improvement programmes in line with evidence based practice and CQC requirements.
- Service activity recording and review
- Ongoing development of service contribution in the light of patient feedback and national policy developments and recommendations
