Our PCN Mental Health Practitioners work within a Primary Care setting and have the full support of Norfolk and Suffolk NHS Foundation Trust. Forming part of the NHS’s long term Community Transformation strategy and driven by the needs of those who use our service, as a PCN Mental Health. Within this role you’ll make a positive impact to the mental health care delivered to our service users by providing a dedicated service that makes sure needs are met by a specialist Mental Health Practitioner in the familiar surroundings of their GP surgery.

As a an autonomous PCN Mental Health Practitioner you will work together with fantastic colleagues to provide assessment, support, treatment, advice, education, therapeutic and problem-solving approaches to members of our community experiencing challenges with their mental health needs. Our Mental Health Practitioners work within our GP surgeries as well as in our community, across beautiful Norfolk and Suffolk. Forming part of a multidisciplinary team, you’ll work together to meet the needs of our service users and enable them to move seamlessly between services.

Primary Care Networks (PCNs) are groups of GP practices who work together in a local geographical area.

As a Mental Health PCN practitioner, you will be responsible for the day-to-day operational and clinical issues arising from within your PCN practice. You will line manage other Mental Health PCN colleagues as part of a multidisciplinary team providing advice, support, and treatment for members of our community experiencing problems with their mental health and wellbeing. You will collaboratively engage with the GP practices within the PCN and the PCN Practitioners to work with the priorities for each local population.

You’ll work at a quick pace, autonomously managing your own clinics and provide an intervention led approach, acting as an incredibly important first point of contact for people who need care and support the most.

We are looking to recruit Mental Health Practitioners with an interest and experience in a mental health setting, to undertake the planning and delivery of direct patient care.

We’re looking for conscientious, kind and compassionate individuals who work well both independently and as part of a team. Driven by the needs of those who use our service, you will need to have good organisational skills and experience in this area as you will be providing services to people with mental health care needs as well as offering support and advice to other health care professionals. You will be an autonomous practitioner, making clinical judgements, identifying alternative courses of action, managing and providing high-quality care.

We are working jointly with our primary care colleagues and as the PCN's evolve, we will consider the wider health of their communities, taking a proactive approach to managing population health.

On a day-to-day basis, all PCN Practitioners will work as a member of the GP practice extended teams and be based in practices. This role will also support secondary mental health services by acting as the interface from primary care to manage referrals into the correct services at the correct times. These roles have already been shown to reduce referrals into secondary care (data collected from practice pilot areas).
