**About Us**:
Complex Case Management is dedicated to providing bespoke and person-centred care services for clients in the community across the Northwest. We are currently recruiting for a compassionate and motivated individual to support a 60-year-old male client with an Acquired Brain Injury (ABI). The role involves providing 1:1 care and rehabilitation support within the client’s home and community.

**Key Responsibilities**:
**Specific Duties**:

- Assist the client with rehabilitation goals, including support with physical therapy and exercise routines.
- Facilitate attendance at therapy appointments (e.g., physiotherapy).
- Provide support with daily activities such as graded cooking tasks, leisure activities (e.g., snowboarding), and maintaining a structured routine.
- Support the client’s health and nutrition, including fatigue management and maintaining general well-being.
- Assist with maintaining the client's safety in various environments and with pet care.
- Contribute to household duties, including cleaning and maintaining a safe and comfortable living environment.
- Ensure the client’s safety and well-being at all times, including during outings and recreational activities.

**General Duties**:

- Promote and maintain health, safety, and security in the workplace.
- Foster effective communication and build positive relationships with the client, their family, and the multidisciplinary team (MDT).
- Contribute to safeguarding efforts, ensuring the client is protected from abuse.
- Build and maintain a trusting, therapeutic relationship with the client.
- Maintain accurate and detailed records, including timesheets, daily notes, medication logs, and critical incident reports.
- Attend team meetings, reviews, and supervisions with the Complex Case Management team, the client’s case manager, and other MDT members.
- Participate in regular training and development sessions to maintain and enhance skills.

**Essential Qualifications and Skills**:

- Previous experience in care, particularly with individuals with Acquired Brain Injury (ABI), is essential.
- A compassionate, enthusiastic, and motivated individual with a positive, professional attitude.
- A minimum of 2 years’ experience in care work, preferably in a rehabilitation or ABI setting.
- Ability to work independently and as part of a multidisciplinary team.
- Excellent communication and interpersonal skills.
- Over 21 years of age with a valid, clean driving licence.
- Enhanced DBS check required.

**Note**: This is a female-only role as requested by the client.

**Working Hours**:

- The role is part-time, requiring 8 hours per week.
- Flexibility is essential, as you will be required to cover weekend shifts on an alternate basis (Saturdays and Sundays), as well as weekdays, weekends, and bank holidays as necessary.
- Hours of work may vary depending on the client’s needs.

**Salary and Benefits**:
Hourly Rate: £15.00 per hour (Weekdays) | £16.00 per hour (Weekends) £17.00 (overtime)

**Application Requirements**:
**Experience**: Minimum of 2 years in a care setting, with specific experience in Acquired Brain Injury care.

Work Authorisation: Must be eligible to work in the United Kingdom.

Shift Availability: Must be able to work alternating sat/sun every weekend and provide additional hours as required (During the week)
