**Key Responsibilities**:

- Deliver exceptional personal care and support to clients.
- Implement care plans and risk assessments to meet clients’ specific needs.
- Maintain accurate and up-to-date records for efficient care coordination.
- Communicate effectively with clients, families, and healthcare professionals.

**Requirements**:

- A minimum of 6 months previous paid experience as a carer or support worker.
- Ability to commit to a minimum of 24 hours of work per week.
- Access to your own car is essential for this role.
