**JOB DESCRPTION**

**Job Summary**:
The post holder will work within the staff team to develop relationships with family’s parents/cares children & young people in the community. The post holder will work in partnership with schools and other relevant services who have identified children and young people who need additional support.

The Post Holder will develop effective relationships with parents and carers via a whole family approach, ensuring positive partnerships between parents /cares, pupils and schools are fostered and developed.

The Family Link Worker will provide direct support to children and their families facing a range of difficulties. With the family and partner agencies you will develop individual targeted plans of intervention in accordance with the needs of the child/ young person and family.

You will advise, assist and support parents to take responsibility for their child’s attendance, behaviour and health needs by building on the strengths of the family in addition to offering practical support where required.

**Main areas of responsibility**
- To work with parents and their children in their home and groups/organisations outside of the home setting
- To increase the skills, confidence and abilities of all family members to live their daily lives in a positive and safe way
- To offer emotional and practical support to parents and their families in their own homes and in the community
- To offer a whole family / family centred approach to families with complex needs and who are identified as part of the Troubled Families cohort and/ or those requiring early help
- To build a trusting relationship with the parents and young people to help alleviate or break their social isolation and increase capacity
- To build up knowledge and understanding of local resources and community and statutory services, including Mental Health Teams, Social Services, Health, CAMHS,

Schools and voluntary services and communicate effectively with them in the best interests of the child and family
- To help plan creative and innovative responses to families’ needs and keep high quality records and assists with the preparation of reports
- To attend conferences, review meetings, supervision and training sessions as requested
- To promote the safeguarding of children and young people in accordance with Priority

Youths Safeguarding Procedures
- To work effectively and cooperatively with partner agencies using common processes including the Common Assessment Framework (CAF), Team Around the Family (TAF), and Lead Professional (LP) roles
- To use an assertive approach to challenge and confront families on negative behaviour and areas they need to improve on, whist being honest and direct.
- Develop clear individual targeted plans of intervention in accordance with the needs of the child / young person and their family as part of the CAF process

To work within, adhere to and understand all PYP policies procedures and good practice guidelines specifically relating to confidentiality, data protection and data sharing protocols
- To support families to develop practical parenting strategies that enhance positive parent - child relationships
- To reduce inequalities in outcomes between the most disadvantaged children and the rest by implementing effective practices in narrowing the gap
- To produce clear plans of support and robust records to demonstrate how outcomes aligned to service priorities have been achieved
- To work flexibly as will be required by the needs of the service and carry out any other

reasonable duties as required
- Develop the PYPS commitment to equal opportunities and to promote non
- discriminatory practices in all aspects of work undertaken
- To promote services for children, young people and families within the local community, councillors and key officers
- To perform and ensure the discharge of administrative duties (including budget control, record keeping, monitoring and health and safety in accordance with the charity’s policies and procedures.

**Work with others**
- To work in partnership with a wide range of statutory and voluntary agencies and children’s services.
- To work in partnerships with local employments agencies/ employment services/ social care teams/ housing support teams
- To develop links with parents, carers and other professionals involved as appropriate.
- To promote a wide range services for children, young people and families with a range of youth work projects and related services i.e counselling, advice and employment services

**Monitoring & Evaluation**
- To provide written reports on request and regularly to Line manager and the Project
