These positions are joint positions, if you are currently on NHS T&C’s you will be able to stay on these terms. If you are a current member of SCC or an external applicant, you will be employed under SCC T&C’S.
**You will be**:
- Leading projects to keep people well at home and maximising their independence through transforming the way we work, building relationships, reducing duplication, unnecessary admissions to hospital and delivering services closer to home.
- Working in a friendly and supportive environment, alongside professionals who are committed to developing the service, such as social care teams, community health teams, statutory services & voluntary organisations, NHS colleagues, and other organisations within their designated locality.
- Collaborate with partners, to design innovative and integrated community services.
