An exciting opportunity has arisen to be part of Humber Teaching NHS Foundation Trust’s new Community Ageing Well Programme. The successful applicant will be appointed as an to support the integrated service delivery for the Virtual Ward service as a ‘Virtual Ward Clerk’
Virtual Wards allow patients to receive the care they need at home, including care homes, safely and conveniently. They also provide an opportunity to narrow the gap between demand and capacity for secondary care beds, by providing an alternative to admission and/or early discharge.
The initial model delivery will be within Scarborough community, with a focus on admission avoidance and early supported discharge for people living with frailty in Scarborough. This is a developing service and is expected to expand to cover the North Yorkshire patch of, Whitby, Scarborough and Ryedale, and Pocklington Community.
The service will operate 7 days a week, for a minimum period of 08:00-20.00. As a developing service, there may be a requirement to extend hours for administrative support beyond 5 days as the service develops.
The successful applicant will provide effective and efficient administrative support to the Virtual Ward service. Furthermore, they will provide clerical support to associated clinical staff. As well as, dealing with queries from patient/visitors/staff.
