**Exciting opportunities have arisen in Preston for full time 37 hours/week permanent Family Support Workers
Family Support Workers are operational frontline delivery staff linked to either 'Family Intensive Support' (FIS) or Neighbourhood Communities arrangements (N/C), distributed across the 5 ICP footprints incorporating the District area groups; Lancaster, Wyre/Fylde, Preston, Chorley/South Ribble, West Lancashire, Hyndburn/ Ribble Valley, Rossendale, Burnley and Pendle.
They will carry a caseload of children and young people in families with unmet needs up to the thresholds for statutory services, with more focus on intensive support (Level 3). As case holders, they will undertake appropriate assessment of the needs of their allocated client group in line with agreed assessment processes and progress appropriate planned responses.
Family Support Workers will undertake the 'Lead Professional' role where appropriate in cases which require multi-agency support for families/individuals. They will develop and deliver smart action plans designed to address identified needs and use agreed tools to measure the progress made by families/individuals towards substantial and sustained change e.g. outcome stars. They will record their work fully and appropriately within agreed systems
Flexible working is required and some involvement in evening and weekend delivery may be required to meet the needs of families.
Family Support Workers will be line managed by the appropriate Senior Family Support Worker
