- Experience in care is preferred but not essential as full training is provided.

**The Job**:

- We are looking for compassionate, reliable people to join our dedicated Hospital Discharge team.
- You will be responsible for assisting individuals on a one-to-one basis in nursing, residential, and care home settings.
- Promoting independence, and supporting them with personal care and social support.

**The hours are as follows**:

- Choose from full/part-time or weekend-only roles from 12 to 44 hours per week on a 12-hour shift system days or nights.

Subject to qualifying criteria.
