**About the Service -Treatment & Recovery Services**

Our dedicated treatment and recovery services offer tailored support to help people challenge dependency. We understand that recovery from dependency is a personal process, and we know that it is possible with the support.

At Changing Lives, we recognise that substance dependency is often something that emerges linked to life experiences and we are here to help people explore, make meaning and decide what changes they would like to make moving forward.

We work with people at every stage of their recovery journey. We offer an effective therapeutic support for people, and on-going support as life progresses. We also support people to connect within their communities and to other services that can help with other issues arising, including employment, housing, family relationships, mental health and more.
**About the Job**

To work within Northumberland Recovery Partnership (NRP) on an assertive outreach basis, engaging with service users who have been identified via the Blue Light, or those who have had frequent hospital admissions with the primary focus being alcohol. This role will include brief advice, harm reduction and motivational work aimed at encouraging and preparing individuals to engage in structured treatment. There will be a flexible and holistically approach to engage people. Clear communication with NRP to ensure people are being referred appropriately and swiftly.

Work collaboratively with services, internally and externally, and to develop effective pathways, to

ensure cohesive and seamless transition for service users. Work within a strengths-based, recovery-orientated, change and outcomes focused approach that promotes service users, families and communities as responsible co-producers of health and well-being. This role may involve one-to-one and group work with clients engaged through outreach as well as those who are deemed hard to engage. The outreach work will cover the whole of Northumberland.
