**Company description**

The role of the support support worker is to advice and assist people who are living with physical/learning disabilities, mental health and health needs.

Promote physical and mental, emotional well-being and enable them to lead their own lives and promote their independence.

Person center care planning and care plans while prompting choice and control for individuals.

Provide a safe working environment, for staff and service users, including risk assessments, health and safety, COSHH and all accepts of manual handling.

We will maintain accurate records of support, condition, progress and concerns.

Respond appropriately to the physical, emotional and development needs of each individual that we support.

We will obtain information about the service user’s medical history, drug history and allergies.

Provide personal care and assistance with medication to service user who require this while maintaining accurate records and daily activity logs.

Liaise with all outside agencies who are involved with the individual.

Ensure staff are fully trained to meet the needs of each individual.

We are recruiting for a full-time and part-time Health Care worker to join our independent domiciliary care and outreach provider in Halifax, West Yorkshire,

**Driver**

Our client’s range in age and support needs with the majority living within their own home. We provide care services for no less than 60 minute visits up to on-going 24-hour care.

**Duties**

Providing care and support

It is difficult to outline every task that a support worker might have to complete. Below is a list of the most common tasks. Be aware that it is not an exhaustive list. Tasks should be completed in a professional and respectful manner whilst prioritizing the dignity of clients.

Shopping - appointments - life skills - days out - socializing in the Community and building friendships with others - Medicine administration, personal care.

At times general household duties - laundry, ironing, light cleaning - Promote client independence and activity appropriate to their abilities - To provide general support to clients as part of a caring team - Within an agreed plan, contribute to the supervision of people who have a tendency to wander, neglect themselves or put themselves at risk

**Communicate effectively**

Support workers must maintain professional relationships with clients and their families and remember that the client’s home is a place of work. Support workers should alert a Service Manager or the Registered Manager if they have any concerns about a client’s condition.

**Training**
- All support workers must attend compulsory training and adhere to Company policies and procedures and training techniques when providing care to clients. Compulsory training includes the induction for new staff and ongoing training for existing staff.

Pay: £11.75 per hour

Expected hours: 37.5 per week
