This vacancy will be working with the recently created discharge to assess team. Working alongside qualified therapists and the wider MDT the role involves assessment and equipment provision for patients in their own homes to provide timely and safe discharge from hospital.
We are looking for an experienced and dynamic Therapy assistant to work with the team to assess, advise and complete onward referrals for a wide variety of patients being discharged from hospital
You should have experience of working in rehabilitation ideally within a NHS setting, be enthusiastic and keen to work with the Therapies team to provide an excellent service.You will need to have good communication and organization skills, be a team worker and able to use your own initiative. It is essential that you are a car driver for this position.
You will be supported by qualified therapists, working within a competency framework and will be given all necessary training for the job. You will manage your own workload and participate in team and service development.
