Support Time Recovery Worker

An exciting opportunity has arisen within the Specialist Care Directorate Community Rehabilitation Team at Hilda Johnson House. This is part of the re-developing Rehabilitation Service and are looking to recruit to a 30 hour post of a band 4 Support Time and Recovery Worker. The role will be dynamic, fast paced, and working with people with complex mental health needs in the community and in their own homes. The purpose of this post is to work as part of an extended MDT focusing on the direct needs of service users, providing intense Support, giving allocated Time to those with more complex needs and thus promote their Recovery enabling community integration. The service is 24 hour service and the post will require working on a rota basis with some nights, evening, weekend and lone working. It will also require the post holder to work autonomously and have the ability to make sound decisions with the support of the senior management team. Therefore we are looking for a self-motivated individual with excellent communication and time management skills who works with a positive approach.
Responsible for providing the link into the care co-ordination process for an allocated number of individual service users. This will include attending and actively participating in training sessions, team/care plan review meetings and supervision as appropriate.
To ensure that service users engage effectively with the agreed Care Plan and access appropriate services provided on a regular and consistent basis.
Positively promote independent living of service users within their community.
Developing a rapport based upon companionship and friendship but within appropriate and transparent boundaries.
Provide regular and practical support to service users and their carers in developing and managing dignity and independence.
Provide support with daily living; “living ordinary lives”
Help service users gain access to resources to include benefits and welfare rights.
Provide information on health promotion.
Help to identify early signs of relapse by monitoring the service user’s progress, level of functioning and mental state and alert the appropriate staff involved in their care.
Report regularly to the Unit Manager and Care Co-ordinators as appropriate key worker(s)
To maintain adequate records as required by existing procedures, entering appropriate details on the service user’s case notes as necessary.
