We are looking to recruit a Frailty Specialist Practitioner to deliver care to the residents of North Dorset. This is an opportunity to be involved in a truly integrated way of working with the Vale PCN to provide high quality proactive care as part of an integrated frailty hub team.

You will be based in the Shaftesbury Hub at Westminster Memorial Hospital in Shaftesbury. You will join a dynamic team of Practitioners, who are supported by The Vale PCN GP's, Social Prescribers and Frailty HCA's. The Hub offers a truly integrated service with Social Services supported by Health and Social Care Co-ordinator's, Integrated Community Rehabilitation and Older Peoples Mental Health.

You will provide both proactive and reactive care to prevent unnecessary hospital admissions by supporting individuals to remain in their own homes by undertaking holistic and generalist assessments at the point of need. You will assist individuals and their carers to proactively anticipate their future health needs, support self-care, develop appropriate plans, manage deterioration via early intervention and work collaboratively with the wider MDT Team to promote improved outcomes and the management of frailty.

The service must be covered 8.30 to 18.00 with a possible future weekend rota as the networks develop.

To provide a high quality person centred approach to care delivery which always considers people’s safety, privacy and dignity.

To provide specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multi-disciplinary care team.

To provide specialist advice to others regarding the management and care of patients / service users.

To devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.

To plan, implement and review health improvement programmes in a range of settings.

The post holder will work closely with the team lead in the day to day running of the service and have some line management responsibility including absence management, appraisals, supervision, service development and other management responsibilities in the absence of the team lead.

Provide supervision, mentorship, training and clinical support to students and trained practitioners.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

This exciting and challenging role offers the post holder the opportunity to provide high quality patient focused care in the community supporting the Vale PCN as they deliver their Carousel and Wellbeing clinics. The role supports the individuals in the community through robust physical assessment skills, clinical judgement, critical analysis and decision-making skills.
