The Primary Care Network Mental Health Team (PCNMHT) is a newly developing service which comprises of three pathways. The aim of the team is to bridge the gap between primary and secondary mental health care: providing accessible primary care level mental health interventions to the population of North Lincolnshire.
**The pathways are**: Routine Pathway, Dementia Pathway and Severe Mental Illness Physical Health Check pathway. The role of a Senior Support Worker within the PCNMHT would involve working across all three pathways, working as part of an integrated team to deliver interventions in line with the primary care model. This includes interventions such as physical health checks and health and wellbeing sessions.
The North Lincolnshire PCNMHT are committed to establishing our service within the wider Primary Care Networks and have colleagues embedded in GP surgeries. Expanding this is the focus of our service. We welcome applicants who feel they have the drive, skills and expertise to continue this further development.
Applicants should have a NVQ Level 3 in care or willingness to work towards the award and previous experience of mental health issues/emotional distress.
This is an exciting opportunity for an experienced mental health professional to join our team as Senior Support Worker.
We are looking to recruit an experienced, enthusiastic and highly organised mental health professional to provide high quality healthcare support under the direction of the Nursing team and Allied Health Professionals for patients within a primary care setting.
The role of a Senior Support Worker within the PCNHMT will require skills in physical health, performing physical observations and physical health checks, as well as skills and experience in working with adults experiencing mental health problems.
This role involves regular travel around the North Lincolnshire area, therefore access to own transport is required.
