**Overview**

**RESPONSIBILITIES**
- To ensure that correspondence and reports generated or

required by clinicians is dealt with and circulated promptly

and efficiently.
- Complete patient referral administration on behalf of

clinicians using eRS and manual referral systems
- Liaise with patients, hospitals, Social and Healthcare

Department, Health Authority and other outside agencies.
- Liaise closely with clinical staff and the reception team to

ensure patients are directed to the appropriate care pathway.
- Monitor referrals made by practice clinicians and initiate

follow up with other healthcare providers where patients

have not received confirmation of ongoing care.
- Help patients to manage their needs through answering

queries, being a first point of contact in the practice for

patients referred to external healthcare providers.
- Maintain and respect patient confidentiality at all times.
- Ensure referral pathways and contact information for

hospitals, Social and Healthcare Department, Health

Authority and other outside agencies is maintained, updated

and easily available to the practice team.
- Arrange and make appointments for Outreach Clinics as

required
- Manage incoming tasks for clinicians from patients and other

agencies, referring queries to clinicians where appropriate

and responding on behalf of clinicians.
- Arrange patient transport/ambulance where necessary.
- Expedite appointments/tests as requested by clinicians.
- Transcription of audio recordings from Lexacom dictation

system to documents.
- Use NHS resources effectively
- Any other tasks allocated by Partners/Managers.
