Are you a Qualified Social Worker with experience in Adults Hospital Social Work?A local authority based in the Northwest (Merseyside area) is giving you the opportunity to join their busy and enthusiastic workforce as part of their Hospital DischargeTeam. You will be working closely with hospital staff in a Multi-Agency Service consisting of nurses, OT, Drs, and support workers to help support patients being discharged to either home or intermediary care. The post will focus on ensuring patients havethe correct package of care necessary for them to safely be discharged.Responsibilities include:Completing Social Care Assessments for residents within hospitalManaging fast-paced work and a caseloadThe social worker will need to complete Care Act assessmentsand long-term support plans, Mental capacity assessments, and Best Interest AssessmentsIdentifying appropriate support for discharge voluntary services; 24 hour placement or domiciliary care packageReviewing support following dischargeThe role allows you flexibilitywith working from home, but you will be required to complete face-to-face assessments and you will often be based in the hospital.
