You will be responsible for coordinating the care of service users under your care, ensuring they are engaged with the MDT and that they are offered the core interventions for those experiencing first episode psychosis. This will be done in collaboration with the Service User, their carers/ family and other professionals/ agencies. The plan will be informed by a comprehensive initial assessment of needs, supplemented and reviewed as necessary during the Service User's stay with the service. The plan will include a risk assessment and a crisis and contingency plan.

The role includes aiding Service Users in accessing opportunities and services which may include housing, benefits, employment, Citizens Advice Bureau, refugee organisations etc. The Care Co-ordinator is integral to the delivery of care and will need to provide regular feedback to the Multidisciplinary Team.
