**Who are we?**

Kibble is Scotland’s specialist provider of services for at risk children and young people. By providing a range of integrated services, we help build positive futures for children and young people, with complex social, emotional, and educational needs.

**Job Purpose**

**Main Duties**
- To meet with survivors to discuss their needs and prepare a support plan with them to help manage community living and connections. Regular check-ins with young people. Depending on distance this could be in-person, via phone or online.
- To regularly review and monitor the support plan with the young person ensuring the support required reduces over time, helping them to eventually live independently.
- To help young people to acquire daily living skills required to enjoy a safe and healthy life.
- Support and signpost young people to access services in their community e.g. register with a doctor, dentist, learn how to use a library.
- Encourage young people to take an active part in activities within their community.
- To contribute to reviews as part of a wider multi-disciplinary team around each young person, where appropriate.
- To support development of wellbeing practices, linking with local services, and managing independent living.
- To link young people to support group/organisations in their community and help them to attend, where practical.
- To assist with the completion of statistical returns, providing information for report writing as required by the funding team.

**Qualifications and Experience**
- Experience of working with young people or providing care/support to vulnerable people is essential.
- Relate to young people and have a proven track record in building and sustaining relationships with them.
- HNC in Social Care, SVQ Level 3 in Care or equivalent or the ability to undertake the HNC and SVQ Level 3 Social Care.
- Possess a full driving licence and be willing and able to drive provided transport A flexible approach to work shift work will be required in order to support the needs of young people. Occasional travel throughout Scotland as required to fulfil the remit of the post.
