Due to our continuing growth, we are looking for a caring and compassionate person, with previous Care/ Care coordinator experience, who is ready for their next challenge.
As a Care Coordinator, reporting to the Registered Manager, you will contribute to the delivery of high-quality care for service users in the social care sector by allocating the right staff to the individual service users in accordance with their specific care needs, monitoring the quality of service by working closely with the service users: management, field care supervisors and care workers.
**Duties will include but not limited to**:
Accepting, allocating and processing referrals for new work efficiently. Accepting and processing changes and amendments to existing care packages.
Assigning care workers to service users, whilst working within the standards and requirements of the company.
Allocating work to Care Workers fairly and appropriately.
Monitoring of sickness, absenteeism, or holidays, arranging appropriate cover.
Induction of new support coordinators
Completion and distribution of Care Worker rotas on a weekly basis.
Maintaining accurate records on the system, working with the office team ensuring that all data is relevant and up to date.
Preparing reports as and when required
Participating in the ‘on call’ on a rostered basis, ensure that all on-call records are completed correctly and all alerts are monitored.
Weekly time sheet auditing
Submission of KPI’s on a weekly basis
Supporting Care Workers throughout their induction period, arranging appropriate shadowing completed.
