Home Care Practitioner - fixed term to cover Maternity Leave hours - 42.50 hours available in total, will consider mixes up to 35.0 hours per week

To carry out tasks as delegated by Registered Practitioners, but staff will be working alone and be expected to feedback appropriate information regarding the patient’s condition. To work as part of the Integrated Care Community Team, providing care for patients in the community setting within the patients home with the aim of facilitating early safe discharge from hospital and preventing hospital admissions / readmissions. This will primarily be a role of supporting and enablement, encouraging as much independence and rehabilitation as possible under the guidance of Registered Practitioner. To carry out diagnostic tests as delegated i.e. Phlebotomy and at home ECGS, and others as required for the role. To maintain and review accurate records to ensure continuity of safe care provision.
To work part of a full multidisciplinary team-providing support with rehabilitation and all aspects of personal care with the main aim where possible to aid independence.
To work under the direction of a qualified practitioner
Contribute to the process of gathering assessment information through observation and communication with patients and carers
Support and assist new members of staff in their induction
The post includes working on a shift basis including evenings, weekends, bank holidays and being able to work across the Carlisle Locality area. shifts 07:00/13:00-16:00 /21:00 20:30/07:00

**Disclosure and Barring Service**:

**Standard Check**: £26 + Administration cost of £4.80 = £30.80. Deducted at £9.80 from 1st month salary and £7 deducted from salary over following 3 months.
**Enhanced Check**: £44 + administration cost of £4.80 = £48.80. Deducted at £15.80 from 1st month salary and £11 deducted from salary over following 3 months.
