Rehabilitation Assistant up to 75 hours available
We have three Rehab Assistant posts to work with Intermediate Care Service patients and provide them with the highest quality care (with a rehabilitation and therapy focus) in order to support them to maximise their recovery potential and enable the best possible quality of health and wellbeing after stroke and generic conditions.
The role of the Intermediate Care Service is to provide specialised rehabilitation to patients within interim 24hr bedded settings, facilitating smooth transfer from Acute and Community Hospital settings. To assist patients to achieve their optimal level of independence and integrate back into their communities with the appropriate level of support for them and their carers. To prevent inappropriate hospital admissions and promote a supported discharge from hospital as early as is appropriate and safe. To provide high quality multi-disciplinary (MDT) assessment and treatment, advice/support and / or referral onto other agencies. The referrals will predominantly be from both the Acute and Community Hospitals, but where appropriate from community settings where is has been recognised that a patient needs to step up into a service such as the Intermediate Care Service to prevent an admission to the Acute Trust.

Successful applicant will be redeployed once winter funding is over
To be able to work unaccompanied as a lone worker, without direct supervision at times but under the professional guidance and supervision of qualified staff.
To contribute to the assessment, implementation, evaluation and modification of team interventions.
Use skills to carry out simple assessments and prescribe routine equipment within the scope of the job and to a level of assessed competence.
Contribute to service development.
To maintain accurate written and computer held records.
Be responsible for daily administration tasks and duties.
To work flexibly to meet the needs of the service and the patients which will include bank holidays and weekend working.

**Standard Check**: £18 + Administration cost of £3.85 = £21.85. Deducted at £6.85 from 1st month salary and £5 deducted from salary over following 3 months.
**Enhanced Check**: £38 + administration cost of £3.90 = £41.90. Deducted at £11.90 from 1st month salary and £10 deducted from salary over following 3 months.
