**Better at Home Waiting Well - Service Coordinator £28,000 per annum pro rata for 17.5 hours per week**
**We will consider applicants who wish to combine this role with the Data Officer role (also being advertised), creating a full-time role at £26,500 pa.**
**Background**
These exciting new roles are being introduced in response to winter pressures on the NHS, workforce challenges and increased pressure on Acute Hospital services to manage the backlog - with a specific focus on patients on long waiting lists. The Waiting Well project team at Reach Out Hertfordshire (part of North Herts and Stevenage CVS) will support a cohort of patients identified by the East and North Herts NHS Trust as being on a long waiting list with community support. The Waiting Well approach has been successfully trialled in West Hertfordshire Hospital Trust (WHHT), which supports this agenda and aligns to existing resources in the community and is now being developed for patients of Lister Hospital, with a view to further developing community resources and support offers.
Both Hertfordshire and West Essex have important infrastructure to support this work, including HertsHelp and the Hospital and Community Navigator Service (Herts) and Frontline and Live Well Link Well in West Essex, creating strong networks in the voluntary and community sector.
This post requires some mínimal travel within Hertfordshire
**Purpose of the Waiting Well Service Coordinator**
The Waiting Well Service Co-ordinator will contribute to reduction of hospital readmissions by ensuring the personalised care offer from social prescribing is fully utilised, by building on the Waiting Well approach supporting patients on a long waiting list with offers of community support.
**Key Responsibilities**
- Line management of the Data Officer, Volunteer Coordinator and Administrator
- Working closely the West Herts Waiting Well project Service Coordinator and staff to deliver a county-wide service.
- Improving data sharing improved in line with appropriate Information Governance regulations
- Developing consistent and effective referral processes and sharing of lists
- To ensure full awareness in the trust of the community and voluntary sector offer and enhance and develop systems to strengthen existing referral pathways, as well as to capture learnings from users experience of discharge.
- Sustainable systems are put in place for the future to make this part of how the hospitals continue to work
- Contribute to the development of targets
- Monitor progress toward targets and where necessary recommend corrective action to ensure that targets are achieved
**General responsibilities**
- Work within agreed procedures, regulations, and systems, including for accounting, expenses, communications, data collection, monitoring, and quality assurance.
- Establish and maintain professional and supportive relationships with colleagues and with local groups.
- Undertake such other work as may reasonably be required from time to time.
**Person specification**
The appointed person must demonstrate and maintain the following knowledge, skills and attributes.
Knowledge
- An excellent grasp of written and spoken English.
- Good knowledge of HCNS’s key stakeholders.
- Good understanding of health and wellbeing and the issues facing adult care and the NHS.
- A good understanding of the HCNS’s structure and purpose.
- Good knowledge of the voluntary, statutory and business sectors in Hertfordshire.
- Familiarity with good practice around data security, privacy and confidentiality.
**Skills**:
- Excellent administrative and ICT skills, with knowledge of MS Office packages and ability to use web based CRM database systems
- Good numeracy, a good grasp of statistics and experience of using spreadsheets.
- Acquiring data from primary and secondary sources and maintain data systems.
- Excellent written and verbal communication, including effective listening and questioning to clarify and interpret expectations, needs and requirements.
- Excellent organisational and time management skills.
- Strong interpersonal skills to enable the development of strong working relationships at all levels.
