The post will support wards to safely plan and discharge patients with the support required.
Discussions with patients and family members and the ability to use and refer to specialists teams in the community as required
We will support with any development staff member needs
We provide in house training relevant to our area.
We can access training from community teams

The role of the discharge Co-ordinator is to provide support to wards, attend board rounds and ensure that all patients are discharged safely with the care they require.
To be able to escalate any delays in care or discharge planning.
To liaise with other team members for patients from care homes and patients that are approaching end of life

- To attend board rounds each day obtaining information in relation to admission reason, current LOS, medical plan and any concerns in relation to discharge.
- To be able to feedback information about your allocated patients to your Discharge Case Manager.
- To chase outstanding investigations which are extending the patients Length of stay.
- Working closely with the ward care co-ordinators
- To be an active member of your allocated team providing relevant information that can be presented to the executives.
- To be able to communicate effectively with colleagues within the team and in the acute ward areas.
- To have an understanding of safeguarding both within the acute and in the community
- To be able to update the current electronic data system daily with patents discharge plans and any outstanding tasks needed
