We have an exciting opportunity for an experienced, dynamic nurse to join the Urgent Community Response Team (UCR) in Rugby. The successful applicant will work alongside existing Clinical Practitioners and Advanced Clinical Practitioners in helping to deliver advanced clinical expertise,
levels of judgement, discretion and decision making in clinical care.
The post holder will be required to manage a busy workload within the urgent unplanned work flow, accepting referrals from a wide range of sources for complex patients to help prevent hospital admissions. In addition, the post holder will be highly visible to the PBT’s providing clinical expertise.
In return the applicant will be supported by a knowledgeable and experienced team with regular clinical supervision together with any relevant training.
We are looking for a competent and organised clinician with nursing experience at band 6 level or above, skills in assessing patients in their own homes, managing the complex care of long-term conditions, Frailty, prevention of hospital admission where appropriate, and supporting end of life patients. The clinician must be able to work closely with medical and nursing staff in Primary and Secondary care whilst autonomously assessing individual patient needs.
The post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care.
To work closely with Medical and Nursing staff in both primary and secondary care, to ensure patients receive appropriate investigation, intervention and treatment with mínimal avoidable delays, whilst improving the quality of care.
To work autonomously assessing individual patient needs, initiating investigations, determine a differential diagnosis and initiating appropriate holistic, research based treatment with the development of a personalised clinical management plan, including independent prescribing.
To put the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, improving the quality of care.
Monitor and lead improvements to standards of care through, supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership.

Maintain personal standards of conduct and behaviour consistent with Trust and NMC/HPC/other professional body guidelines.
Allocate, coordinate, monitor and assess own workload and that of individuals or the service, ensuring resources are deployed appropriately.
Ensure appropriate use of resources in order to meet service/client needs.
Identify the need for change, proactively generate innovations and lead new practice and service design solutions to better meet the needs of the patient and service.
Recognise and act as advocate for patient, carer, service and organisation.
Lead change as required and ensure resolution of conflict.
