Pure Innovations believes that with the right support everyone can live a more connected and fulfilled life. The role of Mental Health Engagement worker is to work with Stockport residents over the age of 18 who have been or are currently users of secondary mental health services. Our aim is to help support people to move away from traditional services by building on their independence and skills within the community. This includes facilitation of Peer Support Groups, Social Activities, Physical Activity, Volunteering and much more. As a bespoke service we aim to provide the right support at the right time to enable people to move on in a supported manner whilst linking into a wider community offer. Sitting with Pure innovations established Wellbeing team allows you to link into the wider Stockport community.
Due to the wide ranging nature of the role you must possess a driving licence and have access to a vehicle for work purposes. You must also have experience of working with vulnerable adults with a similar or comparative role.
**JOB DESCRIPTION**
**Stockport Mental Health Engagement Worker**

**Main purpose of the Job**:
The MH Engagement Worker will provide a range of services to people aged 18+ across the Stockport area who have previously been in Secondary Mental Health care. This can include support around Social Isolation, Group Facilitation, Peer Support Projects, Health & Wellbeing, Leisure activities and community participation and supported onward signposting. You will aim to connect people to their communities in a supported manner to engage in opportunities to contribute to a fulfilled and meaningful life. The post holder will also work with the other members of the MHN Network to meet the service objectives.
**Key Areas of responsibility**
**Responsibilities include but are not limited to**:
1. To plan and prioritise personal targets for each of the clients referred to the service.
2. To be an integral part of Stockport preventative stream as commissioned by SMBC.
3. Support and match clients to suitable activities and opportunities as identified.
4. Creative problem solving.
5. Offer individual support to clients and help manage risk.
6. Create and update personal support plans for each client
7. Liaise with partner organisations and the wider Voluntary and Community Sector organisations within Stockport that may be able to provide additional support to client’s
8. Enhance people’s independence, resilience and confidence through community participation.
9. Work within existing networks and facilitate new contacts to move clients toward independence
10. Identify natural support networks either in the community or in people’s own networks of support
11. Work in partnership with all project partners as part of the MHN.
12. To meet agreed targets as per the MHN and report on these at agreed periods.
13. To keep and maintain records of activity and produce regular monitoring information as required.
14. To participate as appropriate in the monitoring and evaluation of the MHN including associated risks.
15. To work as an active member of the MHN, attend meetings and contribute to the ongoing development of the service
16. To participate in conferences, courses and training events as required
17. To maintain an up to date knowledge of relevant legislation and service developments and to adhere to all appropriate policies, procedures and guidelines
18. To uphold, understand and adhere to all policies and procedures identified by Pure Innovations Ltd.
19. To show a willingness to raise any concerns with management.
20. To Understand and be committed to complying with Pure Innovation’s Equality and Diversity policy.

24. To carry out any other duties as required by the Line Manager
