Cancer Care Coordinator - Band 4 - 1 x 37.5hr Permanent Post
Based at Tameside and Glossop Integrated Care NHS Foundation Trust
The role will involve supporting the Clinical Nurse Specialists with the delivery of the Personalised Care and support, including Treatment Summaries and Health & Wellbeing events.
The role will involve supporting the Clinical Nurse Specialists with the delivery of the Personalised Care and support, including Treatment Summaries and Health & Wellbeing events. Assisting with or providing Holistic Needs Assessments, information and support in outpatient clinics and over the telephone, liaising with all health professionals involved in the patients’ care (including signposting and referrals) and general administration of the service.
Applicants should have a genuine interest in cancer and be able to demonstrate effective communication skills. You will need to demonstrate an understanding of cancer or life limiting conditions. You will need to be flexible, able to use your own initiative, work with a wide range of health professionals and be sensitive to the needs of patients and their families.

The post holder will support the implementation of the Personalised Care Package for people affected by or living with Cancer, including:
- Working closely with the cancer nurse specialists by: - assisting providing electronic Holistic Needs Assessments and producing associated care plans,
- supporting the implementation and administration of Treatment Summaries,
- organising health and well-being events,
- Providing a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs.
- Communicating effectively (daily and directly, face-to-face and telephone) with a range of individuals on various levels, e.g. patients, relatives, carers, other health / social care professionals and community workers,
- Providing general information and support about cancer and cancer services to enable navigation of health and social care and to make choices that are best for the individual’s cancer and their life,
- Promoting equity for patients.
The post holder will be expected to work with all grades of medical and nursing staff and wider health / social care and community teams (including third sector and charity).
