Who we’re looking for
An organised and conscientious person who will manage and support every aspect of a patient’s pathway at offsite providers such as Independent Sector (IS) and other NHS hospitals.
The successful applicant will be able to build and maintain successful relationships and will have strong influencing skills. The successful applicant will have well developed communication skills as there will be significant contact with patients and external providers. Attention to detail will be key as they will be responsible for obtaining consent from patients to be treated at an offsite provider, give information to stakeholders as required, coordinate every aspect of the patient’s offsite pathway, and address any concerns or complaints.
Main duties of the role will be to support and manage the transfer of patients to mutual aid providers, to provide continual updates on progression of patient pathways, and to escalate any potential issues before they arise in a timely manner.
Working with the Operational Management team, Clinicians, and the RTT Tracking Team, they will need to ensure accurate records of the pathway are kept and ensure all parties have carried out their responsibilities in terms of reviewing patients, transferring notes, recording outcomes and organising patient follow-ups.
The post-holder will support in ensuring RTT patient pathways are managed effectively. They will align with the Trust booking teams to ensure process is consistent. They will need to develop effective working relationships with staff including senior managers.
The post-holder will need to develop strong working relationships with a range of off-site providers, to ensure patients are cared for effectively.
The post-holder will need an excellent understanding of PAS, EPR and other medical software systems and be competent with Microsoft packages such as Excel and Word. They will also need to have an excellent understanding of both national and local policies relating to RTT management.

KNOWLEDGE AND UNDERSTANDING
- Have a good understanding of both national and local policies relating to RTT management.
- Be proficient on all patient information systems (including PAS, EPR and other medical software systems as appropriate.
- Function as a reliable and effective source of information relating to all offsite providers, their facilities, performance and quality of care.
PATHWAYS MANAGEMENT
- Keep accurate and detailed records relating to progress along pathways for potentially large volumes of patients.
- Take responsibility for ensuring that key milestones in the care of every patient being treated offsite are met in full e.g., consent for offsite care, transfer of notes, pre-assessment and treatment, effective follow-up.
- Keep accurate records on patient waits to inform Trust RTT Performance Reporting.
