Band 4 Patient Pathway Coordinator (CAT 4 - Gastroenterology)

This is a part-time post for 22.5 hours per week.

Shifts between 8am to 5pm will be required for the role - Friday working day will also be required.

We require a passionate, enthusiastic, reliable and hardworking individual to join our team as a Patient Pathway Coordinator to join our Gastroenterology team.

We are looking for an excellent and proactive team member who can offer commitment in providing support to clinical and non-clinical staff.

You will be the point of contact for all administrative issues relating to patients' pathway of care, working as part of a multi-disciplinary team and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly and within required targets.

This involves booking appointments, managing waiting lists, taking patient phone calls, optimising clinic utilisation, processing clinic letters and other general administrative tasks.

You will provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales.

**The post holder will**:
1/ Be the point of contact for all administrative issues relating to patients’ pathway of care.

2/ Work as part of a multi-disciplinary team and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.

3/ Supervise and allocate work to the APPC team.

4/ Ensure high levels of patient and clinician satisfaction by being an accessible, customer-focused and knowledgeable point of contact.

5/ To provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales.
